The Program Specialist serves as Primary Care Management Module (PCMM) Coordinator for Patient Aligned Care Teams (PACT) across Primary Care services withing the confines of Chalmers P. Wylie Medical Center, including the associated Community Based Outpatient Clinics.
The incumbent is responsible for the overall monitoring of the PCMM program within this Medical Center and will provide the necessary training and guidance to all relevant personnel including clinic chiefs and clinic managers to assure compliance with the National standards and requirements. Duties include, but are not limited to: Analyzes workflow to determine potential problem areas and makes recommendations to correct these areas and to improve coordination between the various units and/or offices Identifies and recommends ways of eliminating, combining, simplifying, or improving procedures and processes. Discharges patients from databases when disenrolled from Primary Care. Maintaining accurate enrollment data. Evaluates, analyzes reports and presents findings related to trends, productivity levels, determinations for staffing ratios, and recommends corrective actions to appropriate personnel for consideration. Provides some necessary financial and budgetary information needed by the Program Analyst and ACOS of Primary Care. Responsible for training employees in the use of some software applications, ensuring competency and compliance.
